Fellowship Application Form
To apply, please download the form below and submit to Gimbel Eye Centre with:
- A curriculum vitae, including an account of the number of types of surgeries you have performed
- A statement of research interests
- At least three names from whom professional letters of reference may be obtained from a source familiar with your clinical and surgical ability.
- Please have these letters sent directly from the referee to Gimbel Eye Centre.
- A letter from you outlining your interest in ophthalmology, your interest in Gimbel Eye Centre fellowship and your expectations of the fellowship program.
